Family History

The family history is of great importance in psychiatry as family relationships, upbringing and dynamics can be closely liked to the aetiology, management and prognosis of may disorders. The detail which is required is greater than most medical histories. In general the family history aims to establish the following: -

  • To ascertain the inherited risk of a psychiatric or medical disorder

  • To understand the family structure, workings and relationships

  • To set the groundwork for the completing the social history and personal history

 

A genogram augmented with a paragraph detailing family dynamics is the easy way to record the information about a patient's family history.

 

Example. Shami
 
 
'Shamis' most enduring family relation is her sister Shibli "we're really close.. she's my role model". Shami describes her relationship with her mother as “up and down but solid” Regarding her father she says; “dads never been around much… he's got a really angry side”

Having been raised by her paternal grandparents from the age of 2yrs – 16yrs she is very close to them. Recently she hasn't see them as often as she would like - “they live in Leeds now I really miss them… and Pa Pa's health hasn't been great”.Neither Shami nor her siblings have any contact with their maternal grandparents after her sister Shibli accused them of physically abusing and imprisoning her. They severed contacts with all the grandchildren. The mother (Sophie) still has some contact with her mother.Sophie and Ravi have a “generally loving, supportive..." marriage. After Sophie had Kavit she was diagnosed with Bipolar disorder and this has caused much disturbance in the upbringing of the children. Its is these hardships that led to the children forming a close sibling bond “we always look out for each other” They see each other weekly  and are “comfortable in each others company”

The suicide of her brother Pavan was after five years of treatment resistant paranoid schizophrenia which really put a strain on all the family “he suffered alot… no one could help him” “but we're getting through it now”'
