The aim of the history is to capture the patients' story and from it derive the necessary information for diagnosis and management. The conventional structure as used by UK psychiatrists is composed of the following headings.

Presenting complaint (+ history)

Past Psychiatric History

Past Medical History

Medications

Family History

Personal History

Social History

Forensic History

Premorbid personality

 

Producing a good write-up is necessary for effective documentation and hence management. When the patients' notes are read by other members of the healthcare team they should portray the patient accurately in an easily readable way. A great way capturing the individuality of the patient is to "quote verbatim" wherever possible through out the history.
