Medications (and allergies)

Here you need to record all current drug intake (prescribed, alternative, over the counter) for both psychiatric and non-psychiatric complaints. Writing down generic names is more useful than trade names. Ask if the patient has their current prescription with them as this will save the hassle of remembering names and doses. Previous medications should also be inquired about.

Any allergies and drug reactions should also be recorded here, remember to note the nature of the patients response as this can vary and has clinical significance.

'Are you currently taking taking a medicines?'

'Do you have injections given to you on a regular basis?'

'Do you take any health supplements? herbal remedies? alternative medicine?'

'Have you ever had a reaction to any medication or anything else?'

Paracetamol 1000mg (when needed)

Drug reactions and Allergies
Haloperidol (tremor, slurred speech, akathesia)
Penicillin (causes sever anaphylaxis)
Shellfish
Wasp stings
